Adults living with a severe and persistent mental illness who are interested in Salus housing must complete an Ottawa Supportive Housing Application Form and submit it to:

Applications must be submitted in person or by mail. Faxed applications are not accepted.

Applicants will then meet with a Salus representative to review eligibility. If all criteria are met, the applicant will be added to the waiting list.

Applicants must:

• Be 16 years of age or older
• Have a severe and persistent mental illness
• Be a Canadian citizen, permanent resident, or have refugee status
• Have a mental health support worker who can provide long-term portable and flexible services

Demand for supportive housing in Ottawa is very high. The current waiting period for housing with Salus is typically 3 to 5 years.

Rent is based on the tenant’s income.

• ODSP: $479 per month
• Ontario Works: $376 per month

These amounts reflect the shelter allowance portion of each program. For tenants with other sources of income, rent is typically set at 30% of income or market rent, depending on the unit.

You can request case management services directly or be referred. Referral forms are available here, at the Salus head office, or through the Canadian Mental Health Association.

After a referral form is received, an intake worker will contact the applicant. Due to high demand, the average wait time to be assigned a case manager is approximately 3 years.
